I have a 97 d90 with a 2" old man emu lift. I was running 265 75 16 BF muds and am looking to run a BF AT 285 75 16 or the bigger 315 75 16. Here is the question: I know that running a 35" tire will off set the stock gearing ratios, and that ideally the gears ought to be changed to accomodate. What happens if I don't. In other words, what are the cons, or problems I will encounter by running a 34.8"X12.3" tire on stock gears, etc.?

On the street Your transmission will be down shifting all the time trying to find the right gearing, and forget about your 4th gear.
Off road you will not have any compression gearing and you have to use your brakes every time you go down hill.

I too run 285, BFG KM MTs. Used to be on stock lift and now on OME 2" lift. No issues except every once in a while I has a slider fender rub under articulation but main because of the offset steels.
You might want too also consider 255/85s. Slightly taller tire and a hair narrower than the 285s.

I just came back from a trip to go wheeling. It was 110 miles of interstate before we got to the trails.
I am running 35x12.50 tires and as of yet the 4.10 gears aren't in so I drove with the 3.54 stock gearing.
Fifth gear is useless unless you have a very good tail wind. And I am serious, we had a 10 mph headwind and of the whole 110 miles I used fifth gear for about three miles. ( downhill only )
So this means running aprox. 60 mph at 2800 rps. I can't wait till the 4.10 gears are in.

Mark, just like Yousef said, with an Auto tranny diving on road the trans will always be trying to find the right gear. If you have a manual trans like Richard or Chris you will be fine as you can hold 1st gear a little more to compensate for the tires but an auto will just be lost.

Is anyone with 285's and 4.1 gear? I wish to hear a feedback.
