Ok for months now, I have been searching and waiting to get a LT230T transfer case and now I'm at my limit and something needs to get done.

Question: I have been under the impression that a '94 d-90 with a r-380 can be fitted with only a LT230(T) case, and that the LT230(Q) will not work.

A friend recently told me that if I changed the input shaft spines to the transfer case I can fit the LT230(Q)?

The LT230Q box will bolt up to your R380 with no problem. The automatics require updating the output shaft when switching the 't' for the 'Q'. The issues you may encounter would be having to remove the interlock solenoid, possibly blanking off some unneeded sensor holes, and possibly changing the front output flange and the rear output housing if the 'q' box in question came out of a DII. But any LT230T or Q will bolt to your R380...it's just a matter of configuring it to your requirements. Just out of curiosity, why is it taking so long to find a t-case?
