Just received several Africa Spec Wheel Carriers that are made by the same company that makes Mantec Carriers. These look the same or very close as I don't have one to compare the new ones to.
I've installed several of these and they operate perfectly. Sorry will not fit on a NAS tailgate without a custom made bracket.

What custom bracket is required? Does anyone have a pic?

Should fit a NAS SW, but for the ST you'll need to modify the supplied bracket to fit around the bends in the tailgate.

Wolf Fab is correct. The supplied bracket will not work with a tailgate as the center of the tailagte is recessed. On a SW the door is flush with the back of the tub. One would have to fabricat a bracket that accounts for the recess of the tailgate skin. Easy to do but not supplied.

Do this mount on existing holes or do you need to drill areas on a ROW truck?

Frame holes are good but you will have the drill into the door and into the tub for the new brackets.

I put one on my 110 and just as an FYI you need to drill the rear crossmember, the body through capping and some holes in door where the existing mount would be. If you have a stock door and not a td5 these work great and are worth it. The design matches the mantec I had. Mantec has since updated the lower hinge mount so it doesn't cover the body. Really just a cosmetic thing.

Here are some photos of mine installed. I had no problem installing it. Just ask Steve how easy it was for me to install. :)
